From 1 July 2025, chronic disease management in Australia transitioned to a single, streamlined model known as the GP Chronic Condition Management Plan (GPCCMP). The plan is developed collaboratively between you and your GP and focuses on the whole person, not just the condition itself.
Your plan may include treatment goals, referrals to allied health professionals, lifestyle support, and regular reviews to monitor your progress. Enhanced nurse involvement provides additional guidance and follow-up to help you achieve the best possible health outcomes.
The GPCCMP is designed to support your medical needs while also considering your emotional wellbeing, lifestyle, family responsibilities, cultural background, and personal priorities.

Your GP will create a personalised management plan outlining your condition, treatment goals, support needs, and any referrals required for allied health services or specialist care.
Our Preventative Health Nursing Team provides additional support, helping you stay engaged with your care plan, monitor progress, and access recommended services.
Your care plan will be reviewed regularly to ensure it remains effective, reflects changes in your condition, and continues to support your health goals.
