The 75+ Preventative Health Assessment is available for eligible patients and provides a comprehensive review of your physical, emotional, and social wellbeing. This assessment may include mobility and balance checks, medication reviews, memory and cognitive health discussions, nutrition, hearing and vision concerns, chronic disease management, and home safety considerations. Our goal is to support healthy ageing through proactive, personalised care tailored to your individual needs.
Your preventative health assessment is designed to help you stay well, maintain independence, and feel supported in your ongoing healthcare needs.
You may be asked to bring a list of current medications, recent test results, and information about any health concerns, support services, or mobility needs.
Your doctor or nurse will review your overall health, daily wellbeing, mobility, medications, and any concerns affecting your quality of life or independence.
We’ll discuss your results and provide recommendations to support healthy ageing, including preventative care, lifestyle advice, referrals, or ongoing monitoring where appropriate.
If ongoing care or follow-up is recommended, we’ll work with you and your support network to help coordinate continued healthcare, preventative screening, and chronic disease management.
