The 715 Health Assessment is available for Aboriginal and Torres Strait Islander peoples of all ages and provides a comprehensive review of your physical, emotional, and social wellbeing. This assessment may include checks for blood pressure, diabetes risk, heart health, kidney health, mental wellbeing, lifestyle factors, vaccinations, and preventative screening. Our team provides culturally respectful, patient-centred care focused on supporting your long-term health and connection to ongoing healthcare services.

What to Expect.

Your 715 Health Assessment is designed to support your overall wellbeing and help identify any health concerns early.

Before your appointment

Bring any current medications, recent test results, and information about your medical history if available. You’re also welcome to discuss any health concerns or goals during your visit.

During the assessment

Your doctor or nurse will review your overall health, lifestyle, medical history, and preventative health needs. This may include health checks, screenings, vaccinations, and discussions around wellbeing and support services.

Personalised preventative care

We’ll provide tailored advice and recommendations to support your ongoing health, including preventative care, chronic disease management, lifestyle support, and referrals if needed.

Ongoing health support

We’re here to support your long-term wellbeing through ongoing healthcare, follow-up appointments, preventative screening, and coordinated care where appropriate.
